Waste Survey Form

Please download and fill out the PDF linked below using Adobe Acrobat DC or Adobe Acrobat Reader DC.  When you are finished, click the Submit button in the PDF to return your data to us via your email client.

1. Download the PDF by clicking the link:  Waste Survey Form.PDF
2. Open the PDF in Adobe Acrobat DC or Adobe Acrobat Reader DC.  Please note that filling out the PDF through your browser will not properly send the submission back to us; it must be completed using an Adobe Acrobat program.  You can download Adobe Acrobat Reader DC for free at https://get.adobe.com/reader/.
3. Fill out the information on the PDF as it pertains to your situation.
4. Click the “Submit” button at the end of the form to email the form back to us.  Select either your default mail client (Outlook, Apple Mail, etc) or an online Webmail client when prompted.
5. An email will be drafted automatically with the completed Waste Survey Form attached.  Add information to your email as needed, and then Send the email to complete your submission.

Alternatively, you can use a Word Document version of our form for your submission.  To submit the Word Document version:

1. Download the Word Document by clicking the link:  Waste Survey Form.doc
2. Open the Word Document with the editor of your choice (Word, Open Office, etc).
3. Fill out the information on the Word Document as it pertains to your situation.
4. Save a copy of your completed form on your computer.
5. Draft an email to us at customerservice@centralohiooil.com, attach your completed form, and Send the email to submit it to us.
